GRCC Medical Release Form
As a precautionary measure in the event of an emergency, a medical release form is required for all minors on all trips or activities in which a parent or guardian will not be physically present. It must be filled it out in its entirety and have a parent/guardian signature. Please make sure that Jonathan has this form and a copy of your primary insurance before the trip/activity.
